Attorneys say Christa Pike is on a ventilator and unconscious after a failed execution in Tennessee on Wednesday.

The stunning malfunction in Tennessee’s execution chamber has intensified scrutiny from both opponents and supporters of the death penalty. Tennessee Gov. Bill Lee has ordered an independent review into what went wrong.

Officials have not released additional details about Pike’s condition beyond attorney statements. The case has prompted renewed calls for transparency about the state’s execution protocols and the procedures used during lethal injections.
